### Definition

The space between a commitment made in a meeting and evidence that anyone will follow up on it — a space created when action items have no named owner, no completion definition, and no escalation mechanism if they stall.

An accountability gap doesn't announce itself. It shows up as the same agenda item appearing in three consecutive meeting notes, each time labeled "in progress," until it silently disappears. Most teams normalize this rather than diagnosing the meeting structure that produces it.

### Indicators

- The same item has appeared in your meeting notes for two or more consecutive weeks without reaching completion.
- Action items from your last three meetings have no named person attached — just 'the team' or 'operations.'
- Nobody has ever asked "What specifically is blocking this?" in a meeting about an overdue item.

### Quick Win

- Open your last three meeting notes.
- Circle every action item still marked "in progress."
- For each action item marked as "in progress," write one sentence: What specifically is blocking it? If you can't answer, that's the accountability gap.
