### Definition

A structured incident review process that identifies root causes in systems and processes rather than attributing failure to individual judgment, producing prevention rather than punishment as its primary output.

A blame-first postmortem produces a better story. A blameless postmortem produces a preventive action. Most organizations default to the first because it feels like accountability, even when it consistently fails to stop the same incident from recurring.

### Indicators

- The last time something went wrong, the first conversation was about who made the decision rather than what made that decision possible.
- You've had the same category of operational failure occur more than once.
- Nobody on your team has asked "What would have had to be different for any person in that situation to have made a different call?"

- Write the first sentence you'd use to open the discussion.
- If it contains a person's name before it contains a fact, rewrite it.
