### Definition

A four-element action item format for postmortems and incident reviews, ensuring every commitment produced by a structured failure review has a root cause, a named corrective action, a responsible owner, and a verifiable evidence standard.

The four-component CARE framework structure:

- **Cause**: The root cause, not the proximate cause, of the failure
- **Action**: The specific change required to prevent recurrence
- **Responsibility**: The named owner who will implement the change
- **Evidence**: The metric or observable outcome that will confirm the change has worked

An action item without a CARE structure is a good intention with a calendar date. Most action items stall not because nobody intended to complete them but because when they stall, nobody knows who to escalate to, how to define completion, or what success looks like.

### Indicators

- An action item from your last meeting has an owner but no stated completion definition.
- When an action item stalls, the default reaction is to reschedule it rather than diagnose the blockage.
- You can't point to a specific metric that will change when this week's most important action item is complete.

2. **Accountable deadline** (a date)
3. **Resulting metric** (what changes when it's done)
4. **Escalation path** (who to tell if it stalls)
