### Definition

The organizational structure in which one person or a small generalist team covers all operational functions is appropriate at an early stage and problematic past approximately 40 to 60 employees or $30M revenue, when the breadth of coverage exceeds any individual's realistic capacity.

Most companies don't choose the generalized COO model. They default to it because it worked for a long time. The problem is that by the time it visibly fails, the failure has usually been accumulating for a year — response times slowing, error rates rising, and the single generalist burning out. 

Naming the model makes the transition away from it predictable rather than reactive.

- Your operations leader is responsible for more than four distinct functional domains simultaneously.
- Operational response times, error rates, or vendor performance have declined without any single identifiable cause.
- Your operations function's structure hasn't materially changed since the company was half its current size.

- More than 20 cross-functional requests routed through one person per month is the signal that functional specialization is overdue.
