### Definition

The organizational failure pattern that emerges when a company crosses approximately 25 employees is characterized by duplicated work, contradictory authority, and disconnected new hires caused by informal coordination systems designed for a smaller company that have outgrown their effective range.

At 25 employees, the founder or senior leader is no longer physically proximate to every decision. Communication pathways multiply faster than headcount.

The informal systems that worked with 10 people — proximity, shared context, and easy escalation, begin failing quietly before they fail visibly. Naming the threshold makes it predictable.
