Definition

An operational breakdown that occurs at the transition point between two teams or two individuals, typically caused by unclear authority over who owns the handoff itself and the absence of documented criteria for when the transfer is complete.

Most operational failures are not caused by incompetence within a team. They are caused by gaps between teams. The hand-off point is where documentation ends, decision authority is ambiguous, and the assumption that "the other team has it" can go unchallenged for days.

Indicators

• A specific recurring failure point in your operation consistently happens at the boundary between two teams.
• No written hand-off criteria exist for your highest frequency cross-team transfer.
• When a cross-team issue surfaces, the first conversation is usually about which team should have caught it rather than how to fix it and prevent recurrence.
